Why Do Product Launches Fail?

These are notes I made on an article within the April 2011 Harvard Business Review. The entire magazine was called "the failure issue" and explored the significance of failure, and the importance of learning from it.
This particular article looked at some of the basic flaws that can result in a product launch not being successful, and why (in some cases) these failures could have been avoided.
